All records requests require a non-refundable $15 processing fee. Payments can be made via cash, check, or credit/debit card.

 

Request for a Fire Incident Report

Please submit a Records Request Form via mail, email, or in person at the District office. In your request, please be as thorough as possible in order to ensure prompt response.

 

Request for Patient Care Report or Medical Billing Records

If the requestor is the person/patient involved in the incident:

  • Complete the Authorization for Release of Patient Care Records form
  • Forms can be mailed, emailed, or returned in person
  • If the requestor is the person/patient involved in the incident no supporting documents are needed.
  • You will be contacted when the records are ready for release, if they are unable to be located, or if more information is needed.
  • Records will be released as noted in the Authorization Form.

If the requestor is a third party (not the person/patient involved in the incident):

  • Complete the Authorization for Release of Patient Care Records form
  • Forms can be mailed, emailed, or returned in person.
  • Provide HIPAA compliant supporting documentation (this allows the Fire Department to legally release the Patient Care Report to a third party).
  • You will be contacted when the records are ready for release, if they are unable to be located, or if more information is needed.
  • Records will be released as noted in the Authorization Form.
